About the Event

What happens when a patient burdened with multiple chronic conditions such as diabetes, hypertension, dyslipidemia, fatty liver disease, and early-stage kidney dysfunction decides to take a different path?

Join Dr. Leonard Leng for an interactive discussion centered on a real-life patient story. Using this powerful case study as our starting point, we will trace the journey from poor disease control to remission, illustrated by clear lab trends. Together, we'll explore how focusing on healthy eating, exercise, and stress management made a measurable difference.

This session is designed for shared learning. You will gain insights into practical, actionable strategies and explore a fresh perspective on the potential of primary care to redefine chronic disease management. Let's discuss what's possible and reimagine chronic disease care together, one lifestyle at a time.

Recording of this talk after the event is available only for SGLM members.
