IBLM Exam (2026) Registration Form

This form is administered through the Singaporean Society of Lifestyle Medicine (SGLM). Registration and prerequisites submission (except for 20h CME Event) close on 15 October 2026.

If you are only able to submit the 20-Hour CME Event proof after this form submission, please email the document to info@lifestylemedicine.org.sg, together with your full name, degree, email address and phone number by 29 November 2026.

Payment

Registration is confirmed only upon full payment. The registration fee is non-refundable. SGLM Members receive 10% discount on the exam fee.
